Lot 1: Medicines (medicines manufactured in foreign countries are accepted, except those of Chinese and Indian origin)

Lot 2: Medical Consumables

Lot 3: Laboratory Supplies and Reagents

Lot 4: Medical Equipment (including installation, maintenance, calibration and after-sales services)

Inclusion in the roster does not guarantee any purchase; each procurement will be conducted through a Request for Quotation (RFQ) among the pre-qualified suppliers of the relevant Lot.

Lot 4: Tıbbi Ekipman (kurulum, bakım, kalibrasyon ve satış sonrası hizmetler dahil)

Listeye dahil edilmek herhangi bir satın alımı garanti etmez; her satın alma, ilgili Lot’un ön yeterliliği onaylanmış tedarikçileri arasında Fiyat Teklifi Talebi (RFQ) yoluyla yürütülecektir.

By emailing the application to the following e-mail: you shall email your application to [email protected], in the subject field, state the following:

a.      “Request for Pre-Qualification # PQ-26-0002”.

b.      Name of Company

c.       Lot number(s) applied for (e.g., Lot 1, Lot 4).

d.      Number of emails that are sent, e.g., 1 of 3, 2 of 3, 3 of 3.

You have to send the pre-qualification files to the email mentioned above after filling in the required info, signing, and stamping them.

You shall not use an external link to submit your application, such as an online drive; all pre-qualification documents shall be attached to your emails. Documents submitted via an external link (online drive) will not be considered in evaluation.

2.2  Only the offers submitted through the above mentioned secure email will be accepted.

2.3  All the pre-qualification documents shall be signed and stamped (sign and stamp each single page). Signing and stamping constitutes the applicant’s unconditional acceptance of all terms of the pre-qualification document.

2.4  Submitting any unclear or inconspicuous documents may lead to these documents not being considered or being excluded from any evaluation.

2.5  No prices are required at this pre-qualification stage; prices will be requested through RFQs issued to the pre-qualified suppliers.

Ön yeterlilik dosyalarını yukarıda belirtilen e-posta adresine gerekli bilgileri doldurup imzalayıp kaşeledikten sonra göndermeniz gerekmektedir.

Başvurunuzu göndermek için çevrimiçi sürücü gibi harici bir bağlantı kullanmayınız; tüm ön yeterlilik belgeleri e-postalarınıza eklenmelidir. Harici bağlantı (çevrimiçi sürücü) ile gönderilen belgeler değerlendirmede dikkate alınmayacaktır.

2.2 Yalnızca yukarıda belirtilen güvenli e-posta aracılığıyla gönderilen teklifler kabul edilecektir.

2.3 Tüm ön yeterlilik belgeleri imzalanacak ve kaşelenecektir (her bir sayfayı imzalayıp kaşeleyin). İmza ve kaşe, başvuru sahibinin ön yeterlilik belgesinin tüm şartlarını koşulsuz kabul ettiği anlamına gelir.

2.4 Belirsiz veya göze çarpmayan herhangi bir belgenin sunulması, bu belgelerin dikkate alınmamasına veya değerlendirmeden çıkarılmasına neden olabilir.

2.5 Bu ön yeterlilik aşamasında fiyat istenmemektedir; fiyatlar, ön yeterliliği onaylanan tedarikçilere gönderilecek Fiyat Teklifi Talepleri (RFQ) aracılığıyla istenecektir.
